Please find the basic forms you will need to complete prior to your first visit. Although these are provided at your first meeting, you may complete these forms at your leisure prior to coming in.

Third Party Payor Agreement
This form is completed by the party responsible for payment to Dr. Boyers if the patient will not be paying for his or her treatment directly. The most common example is when parents are assuming financial responsibility for a child's (who is over 18) treatment. In addition, please find the form that will authorize Dr. Boyers to charge your credit card should you choose that form of payment.
